■Application Form
All participants in scuba diving will be required to fill out an application form.
Even among friends or close acquaintances, always confirm participation in writing.
There is a possibility that decompression sickness or eardrum injuries may develop a few days after completing the dive.
To prepare for any eventuality, keep the scuba diving participation application form for at least 5 years, which is the statute of limitations for liability in Japan's civil litigation law.
Using the STARS application form (PDF file) is convenient.

■Example of Application Form
Let's review how to fill out the form and ensure that no fields are left blank.
1.If the person planning to participate in diving is under 18 years old, obtain consent from their guardian.
Be sure to obtain a signature.
2.Confirm that participants are in good health.
Be sure to obtain a signature.
3.
For those with health concerns, before participating in diving, have them undergo a medical examination using the DAN-JAPANissued medical check form and obtain permission from a doctor for diving.
The medical check form can be downloaded and used from the website.
Participants can download the form from here, instructors can download it from here, and doctors can download it from here.
Let's exclude individuals without a doctor's permission from participating in scuba diving.
Participants who must be excluded from diving for health reasons should not be allowed to participate in diving activities.
People who have not undergone regular health check-ups may not be aware of any abnormalities in their health. Therefore, it is recommended that they undergo regular health check-ups.
4.Let's confirm the diving participants' comfort in water, diving experience, physical fitness, etc., at the time of diving registration to ensure they are suitable for the planned diving.
Check the interviews with the participants and the records in their logbooks to ensure that they can safely participate in the planned diving.
Let's confirm.
5.If the diving participant's comfort in water, diving experience, or physical fitness for the planned diving point is in doubt, it is advisable to recommend participation in a check-out dive (pre-test) to assess their skills.
Especially for participants who have not dived for more than two months, it is strongly recommended to participate in a refresher dive.
A long break can lead to panic underwater.
6.
If it is determined that a participant cannot safely participate in planned diving activities after a refresher dive, or if they are unable to participate in the refresher dive, propose a beginner-level diving schedule appropriate for their skills and allow them to participate in low-risk fun dives.
In team diving, an individual's trouble can compromise the safety of the entire team. Avoid the notion that "it's okay to have a few inexperienced divers." Always conduct refresher dives for participants with unclear skills and provide diving options based on their abilities.
7.
In case of potential changes to the diving plan due to weather conditions or other factors, it is important to notify participants in advance of the alternative diving plan and the method of notification in case of changes. Obtain consent from participants for the alternative plan.
Participants may not be willing to participate in diving at alternative locations. To prevent issues on-site, notify participants of the alternative diving plan in advance and obtain their consent.
Let's get a consent.
8.Announcement: Diving will be conducted using the instructor-specified decompression tables or dive computers. Please note that ascent rates and displays on dive computers may vary by model. To ensure that alarms do not sound during ascent, please verify the prescribed safe ascent rate on participants' computers beforehand.
Be sure to get signatures.
9.Prepare to bring the application form with emergency contact information for all diving participants to the site. Confirm that emergency contact information is provided in case of accidents or other emergencies, and if not, confirm with participants beforehand.
Be sure to get signatures.
■Departure preparation
①Make reservations for local facilities and accommodations.
②Before departing and on the day of departure, check the latest weather
forecast and confirm the local conditions.
It's convenient to check the latest weather forecast on the internet.
The size of the waves is determined by the direction and speed of the wind,
so pay attention to the wind direction
and speed around the diving point.
③Check that rental equipment fits the members properly and is functioning
properly.
For equipment that has not been used for a long time, ensure that there
is no deterioration of the rubber parts
due to ultraviolet rays.
Additionally, it is not allowed to use air tanks that have not passed the statutory inspection.
